Castle North MS Squires Athletic Information

Welcome to the CNMS Athletic Department home page. CNMS currently offers 7 different sports: Volleyball, Football, Basketball, Wrestling, Track, Cheerleading, and Dance. CNMS has 24 separate sports teams that compete at the 6th, 7th and 8th grade levels which allows a variety of athletic opportunities for the students at CNMS.

PHYSICALS

ALL STUDENTS MUST HAVE A PHYSICAL AND ALL OTHER NECESSARY FORMS ON FILE IN THE ATHLETIC OFFICE OR TURNED IN TO THEIR COACH PRIOR TO TRYOUTS. ATHLETES WILL NOT BE ALLOWED TO TRYOUT IF THE PHYSICAL AND ACCOMPANYING FORMS HAVE NOT BEEN TURNED IN.

PHYSICALS MUST BE DATED ON OR AFTER APRIL 1 TO BE VALID FOR THE UPCOMING SCHOOL YEAR.

ONCE THE PHYSICAL AND OTHER FORMS ARE ON FILE, YOUR CHILD MAY TRYOUT FOR ONE OR MULTIPLE SPORTS TEAMS AT CNMS.

Castle North Middle School has updated its policy for the collection of code of conduct and concussion forms, along with athlete emergency medical information.  These items are required in addition to the IHSAA Physical Examination Forms.

Items needed to participate in athletics at Castle North Middle School :

  1. IHSAA Physical Examination Form — must have paper copy
  2. IHSAA History Form – must have paper copy
  3. Consent and Release Certificate – must have paper copy

  5. Warrick County School Corporation Code of Conduct – COMPLETE ONLINE
  6. Concussion and Sudden Cardiac Arrest – COMPLETE ONLINE
  7. Emergency Medical Information – COMPLETE ONLINE

You will need your student’s first and last name, birth date, and Warrick County School ID number.  The School ID number is the 6-digit number students use as their lunch pin or computer login.

AS A REMINDER, CASTLE NORTH MIDDLE SCHOOL WILL ONLY ACCEPT IHSAA PHYSICAL FORMS.

You may also pick up a copy of the Physical and Code of Conduct forms in the main office at CNMS during school hours. If the school is closed, there will be forms available at the Warrick County School Corporation Central Office, which is located in Boonville at 300 E. Gum St.
